If you were recently in the hospital or emergency room, you may be instructed to follow up with your cardiologist. When you call our office to schedule that follow up appointment, here are a few items to know:

*  Please let us know which hospital or emergency you were at, and   

    the date(s) you were there. This will allow us sufficient time to   

    obtain  records that are relevant to your cardiac condition prior to 

    your appointment.
*  Bring your discharge paperwork with you to your appointment.
*  Bring your medications with you to your appointment. Often, 

    medications are changed by a doctor during your hospital stay, so 

    we need to make sure that you resume the appropriate medication              regimen.
*  Come prepared to your visit by writing down any questions you or                 your family may have regarding your cardiac care so that we can                     address your concerns while at our office.
